Insurance rarely covers the cost of evaluation and even less frequently report writing. If your evaluation isn't something we can bill insurance for, it will need to be an out-of-pocket expenses. However, we are happy to provide you with anything you need, such as receipts of payment for services, so that you can request reimbursement from your insurance provider. Calling the benefits number on the back of the insurance card, you can ask if these services are covered and how to submit the bill to them for reimbursement.
